CASE MANAGEMENT
Project HEAL’s Case Management program was created in direct response to the needs of individuals actively struggling with eating disorders. We recognize that navigating care systems, applications, and healing pathways can be incredibly overwhelming, especially while dealing with the cognitive and emotional burdens that often accompany eating disorders (ex. brain fog, fatigue, and confusion).

About Case Management with Project HEAL
What is the Case Management Program?
This program is designed to offer short-term, focused navigational and supportive services to help people move toward the next step in their care or healing journey. It is not intended to serve as an ongoing, long-term relationship, but rather as a bridge to more stable and sustained resources.
Through this program, our aim is to offer clarity, reduce overwhelm, and empower individuals to take meaningful action toward healing.
Case Management is not direct therapy.
Services Offered
Through Case Management, you can receive two core types of support:
Application Support: Assistance in completing applications for Project HEAL programs and other relevant external services or funding sources.
Resource Navigation: Guidance in navigating care options, identifying next steps, and connecting with appropriate providers or resources.
For those seeking resource navigation, this program can assist in researching and connecting with the following resources:
Food pantries
Housing support
Government benefits
Providers under your insurance plan
Free or low-cost clinical assessments

If you are in need of crisis services, please text HEALING to 741741 to be connected to a crisis counselor.
How this program works
Complete a Case Management Intake Form.
You will receive an email outreach from a Case Management representative within 5 business days.
This Project HEAL staff member will offer the option for you to communicate through email or schedule an appointment to chat 1:1.*
Based on the needs expressed, your Case Management representative will assist you in completing the Treatment Access application, navigating to appropriate resources, or exploring Project HEAL’s programs.
You will receive a closeout email with guidance and resource referrals.
Complete our Case Management Client Experience Survey, or sign-up for another call for additional support.
*Pending the availability of the team, the next appointment may be 2-3 weeks out.
Case Management is not ongoing. Applicants can hold up to three (3) calls with a Case Management representative before their case is closed.
After this, individuals may submit additional intake forms for Case Management up to twice a year.

Sign-ups for Case Management are responded to on a first-come, first-served basis.
We encourage applicants to share what systemic barriers you face in your application so our team can tailor their responses and resources to your specific needs and requests.
